Program Features:

• Comprehensive Initial Assessment: Identifies the beneficiary’s needs and determines the level of assistance required.
• Individualized Care Plan: Designed to suit the beneficiary’s needs and individual circumstances.
• Personal Hygiene Care: Includes assistance with bathing, personal hygiene, and changing clothes.
• Repositioning Assistance: Provides the necessary support when changing the beneficiary’s position in bed.
• Safe Transfer Assistance: Supports the beneficiary when transferring between the bed, wheelchair, or other appropriate locations, according to their needs and the available assistance resources.
• Assistance with Assistive Devices: Provides support in using assistive devices according to the beneficiary’s needs.
• Diaper Changing When Needed: Provided as part of personal care services.
• Follow-up and Outcome Evaluation: Reviews the beneficiary’s progress and assesses the suitability of the care services provided.

Qualified Care Provider Team: Consists of two male or female care providers, depending on the beneficiary’s needs.
